The service inquiry into the death of 2nd Lt Max George have recommended 48 recommendations to enhance safety for personnel using the Warrior. 2nd Lt Max George was a member of the 5th Bn The Rifles and was killed by a Warrior Personnel Carrier reversed over him at speed during ‘Exercise Combat Spirit’ (21st June 2022) on Salisbury Plain training area in Wiltshire as he was trying to exit the vehicle through the rear door.

It was found that the emergency alarm was not working on the Warrior. In the report, a number of similar accidents were highlighted that also resulted in fatalities during 1998 & 1999. The alarm on the Warrior involved in the death of 2nd Lt George had been reported as faulty back in 2018, but never fixed.

With the alarm not working, the student Warrior commander ordered the driver to reverse at speed, knocking 2nd Lt George to the ground. The inquiry that took 11months found numerous factors, including some areas of training contributed to the fatal accident.
